Who should take the survey? (who is "eligible" for the survey)
Young people ages 12-25 who are eligible for Nooksack Way of Life programs, activities and Tribal services, AND who live in the local community (Whatcom and Skagit Counties). This includes:

Young people enrolled in the Nooksack Indian Tribe

Direct descendants of Nooksack Tribal members

Adopted children of Nooksack Tribal members

Nooksack tribal employees' children
Each person can only take the survey ONE TIME per year. The survey will be offered in December of each year from 2017 - 2020.
How long does it take?
Some people may be asked more questions (for example, older people are asked more questions than younger people), and it may depend on your reading or internet connection. Most people complete the survey in less than 20 minutes. Please allow about 30 minutes to be safe.
If you leave the survey before it is done you will have to start over in order to get the gift card.
What do I get for taking the survey?
If you are eligible to take the survey (see above) you will receive a $25 gift card as thanks for providing your information. You can pick from Amazon, Target and Visa gift cards.
What if I take the survey, but I'm not in one of the groups above?
If you take the survey but are not eligible, you will not receive a gift card.

I took the survey. How do I get my gift card?
At the end of the survey you should have seen a link that took you to the gift card form. This form asks for your name and an email, cell phone or physical address, but this information is NOT connected to your survey results. The information is used ONLY to send you a gift card and to notify you about the survey next year.
If you choose a digital Amazon or Target gift card, it will be will be emailed or texted to you within 2 days of doing the survey.
If you choose the Visa gift card, it will be mailed to your address within 10 days.
What if I lose my gift card?
PLEASE do not lose your gift card! We are not able to replace lost or stolen gift cards.
What questions are on the survey?
Question topics include:

Age, gender, and race or ethnicity. We also ask for initials as a way of matching surveys from individual youth year-to-year, but not to identify the youth.

Feelings about school and community.

Relationships with parents, friends, and neighbors.

Mental health, including depression and suicide awareness.

Attitudes about and the use of tobacco, alcohol and other drugs.

Participation in traditional or cultural events and feelings about being a Native.
Do I have to do the survey?
No! It is totally voluntary. People taking the survey can skip questions and stop taking the survey at any time.
Does whether I do the survey make any difference in what kinds of Nooksack Way of Life activities I can do?
No! The Nooksack Way of Life activities are available to all young people in the community, whether or not they do the survey.
How will the survey results be used?
The results will help us better understand our youth and provide services that they need. Results will be used in planning and evaluating our Nooksack Indian Tribe’s youth substance abuse prevention programs.
How is my identity protected?
The Nooksack Way of Life Survey is voluntary and anonymous. People will NOT put their name on the survey. Reports of results will not identify anyone individually.
