Why do we need a survey?
Planning for effective substance abuse prevention among the Tribe’s young people
The Nooksack Way of Life program is asking the Tribe’s young people ages 12-25 to take an online survey that will help to plan and evaluate substance abuse prevention and other supportive programs. The survey asks questions about attitudes and use of alcohol, tobacco and other drugs, mental health, feelings about school and the community, and participation in traditional or cultural events and feelings about being Indian. The program hopes to collect 250 total surveys.
The survey is confidential – we are asking for initials so that survey results can be linked from year-to- year, but this information will not be used to identify anyone individually. A separate form, not connected to survey data, will ask for a name and contact information so that we can track who has participated and provide a small gift card as a thank-you. The survey and additional information about the survey will be available from the Nooksack Way of Life program. The Tribe’s young people will be asked to take the survey once per year, between 2016-2020.

Who will be asked to take the survey?
Young people in the Nooksack Community, ages 12-25.
What questions are on the survey?
Question topics include:

Age, gender, and race or ethnicity. We also ask for initials as a way of matching surveys from individual youth year-to-year, but not to identify the youth.

Feelings about school and community.

Relationships with parents, friends, and neighbors.

Mental health, including depression and suicide awareness.

Attitudes about and the use of tobacco, alcohol and other drugs.

Participation in traditional or cultural events and feelings about being a Native.
Is the survey voluntary?
Yes! People taking the survey can skip questions and stop taking the survey at any time.
How will the survey results be used?
The results will help us better understand our youth and provide services that they need. Results will be used in planning and evaluating our Nooksack Indian Tribe’s youth substance abuse prevention programs.
How is personal identity protected?
The Nooksack Way of Life Survey is voluntary and anonymous. People will NOT put their name on the survey. Survey reports of results will not identify anyone individually.
At the end of the survey, people will follow a link to a separate form that is NOT connected to survey results - people are asked to provide their name and contact information on this form in order to receive a gift card.
